วัตถุประสงค์โครงการ
      
   แบบสอบถามการประเมินผล ผู้ผ่านการอบรมหลักสูตรการพัฒนาคุณภาพการดูแลรักษาผู้ติดเชื้อเอชไอวี/เอดส์      
   		  

คำชี้แจง
แบบสอบถาม แบ่งออกเป็น 5 ส่วน ประกอบด้วย
1. ข้อมูลทั่วไป
2. การประเมินความรู้ ความเข้าใจ และการนำความรู้ไปใช้
3. ประเมินทักษะการเป็นพี่เลี้ยงหรือการเป็นที่ปรึกษาด้านการพัฒนาคุณภาพของผู้ผ่านการอบรม
4. การประเมินการเปลี่ยนแปลงที่เกิดขึ้นหลังการอบรม
5. ความคิดเห็นและข้อเสนอแนะต่อการอบรม QI Training
       ผลการประเมิน จะถูกนำเสนอเป็นภาพรวมต่อผู้บริหาร  หลังจากท่านทำแบบสอบถามเสร็จเรียบร้อยแล้ว ขอให้ท่านลงชื่อและที่อยู่ เพื่อชิงรางวัล
โดยชื่อและที่อยู่ที่ท่านได้แจ้งไว้ ขอรับรองว่าจะไม่สามารถนำมาเชื่อมโยงกับข้อมูลการทำแบบสอบถามนี้

ส่วนที่ 2 การประเมินความรู้ ความเข้าใจ และการนำความรู้ไปใช้

5. หลักการที่สำคัญของ HIVQUAL คือความสมดุลระหว่างการวัดผลการปฏิบัติงาน และการทำกิจกรรมปรับปรุงคุณภาพ
